Pathologic 3 Guide: How to Unlock All Endings

Pathologic 3 is a dark narrative game about choice and the price of intervention, where literally every decision you make impacts the fate of the city, its inhabitants, and even the Bachelor himself, Daniil Dankovsky. The storyline once again centers on a city mired in an epidemic, focusing on moral dilemmas and a constant shortage of time and resources. Ultimately, you’re forced to make decisions without definitive answers, only to reap the consequences.

Pathologic 3 offers eight different endings, each reflecting your chosen path, priorities, and compromises. Below is a detailed description of all possible endings and the conditions for unlocking them.

Ending #1 – Seeker of Miracles

  • Achievement “Seeker of Wonders” : Find an unexpected ending that is broader than it seemed.

This ending is considered the basic one, and it is often what gamers will automatically receive if they follow the path of understanding the miracle and do not actively interfere with key forks.

An important condition here is visiting the Polyhedron on the twelfth day and comprehending Simon Cain’s secret knowledge, all his lessons from George Cain. It is this moment that effectively makes Dankovsky a seeker of miracles, even if subsequent decisions prove contradictory.

Visiting the cursed street of Yulia Lyuricheva in Stvorki is formally stated as a condition, but in practice, it has little impact on the outcome. If the Bachelor entered the Polyhedron on the final day and learned Simon Cain’s secret, the game almost always unlocks this ending.

Additional factors that may also lead to “Miracle Seeker”:

  • the death of Mark Karminsky,
  • ignoring Aglaya Lilich’s actions on the second day,
  • a conscious choice to destroy the Polyhedron after visiting it.

In other words, if Dankovsky touched a miracle but was unable (or unwilling) to protect it, the game interprets this as the path of an observer and researcher, not a panacea.

Ending #2 – True Immortality

  • Achievement “True Immortality” : achieve the ending that Bachelor Dankovsky dreamed of.

This ending is considered the most positive and requires the most conscious action. , it can only be achieved after the first playthrough, as one of the key quests is unlocked only after completing any other ending.

The core here, as in the standard “Seeker of Miracles” ending, remains the visit to the Polyhedron on the twelfth day and the unraveling of Simon Cain’s secret. However, from there, fundamental differences begin.

First, Inquisitor Mark Karminsky must survive. To do so, the Bachelor must warn him of Stamatin’s impending assassination attempt on the game’s final day.

Secondly, you need to stop Aglaya Lilich on the second day. After completing the first ending, you’ll be able to go back in time—you need to jump straight to the second day, where the corresponding secondary quest will activate.

A critical point: you must immediately mix the tobacco and belladonna in the morning of this day. Failure to do so early in the day may cause the quest to stall.

Tobacco is regularly traded with children, and the best place to find belladonna is in the nightstand in Lara Ravel’s house, although it can sometimes be found in other cabinets or even in suitcases outside. In an emergency, the quest can be saved through the console. To do this, enter the commands Ctrl+Shift+` – Console. add_item Belladonna_Item.def and add_item Tabacco_Item.def.

Completing the quest with the archive of the first outbreak of Peschanka is also required – without it, the finale will not count.

At the very denouement, Dankovsky must consciously choose to save the Polyhedron, and this choice is only possible if the secret of Simon Cain has been learned, and not erased.

Ending #3 – The Price of a Miracle

  • Achievement “The Price of a Miracle” : achieve the ending that Bachelor Dankovsky was ready for.

This ending largely repeats the path to “True Immortality,” but with a fundamental philosophical difference: Bachelor Daniil Dankovsky renounces not the miracle, but the knowledge of it.

Here, one cannot visit the Polyhedron on the twelfth day and cannot retain the revelation of Simon Cain. However, the other conditions remain the same:

  • Inquisitor Mark Karminsky must survive,
  • Aglaya Lilich was stopped on the second day,
  • the quest with the archive of the first outbreak of Peschanka has been completed ,
  • In the finale, the option of saving the Polyhedron was chosen.

If Simon’s secret has already been revealed, it must be erased from memory. To do this, you must contact the Rat Prophet on the twelfth day, but only if you have previously completed Katerina Saburova’s quest to find him on the fifth day.

It’s important to note that the Rat Prophet will refuse to help if the “Deratization” decree (rodent control) is active. Fortunately, this decree can be revoked at any time. As a result, the prophet erases knowledge of Simon Cain, allowing the Bachelor to save the Polyhedron without the burden of understanding the miracle. This is why the ending can be considered “conditionally good”—the world is saved, but the price of this salvation is high.

To clear knowledge about Simon Cain, enter the command set_global_variable Know_Main_YouAreSimon false in the console.

Rat Prophet

 

The path to the Rat Prophet opens on the fifth day and is tied to a side quest in the hospital . After visiting there, first talk to Dot—he’ll complain about the missing morphine and suspect Clara of the theft. Clara herself is somewhere in the hospital building, but she refuses to discuss the morphine.

When you leave the hospital, Kapella will stop you and tell you how to get her to talk: Dankovsky should be in an unstable state – almost on the verge of mania or apathy.

After this, head to Stamatin’s “Broken Heart” tavern in Zavody. Talk to the twins at the bar—they’ll confirm that Clara only confides in those who are “out of their minds.” Clara is sitting on the stage, but will only begin speaking when her mania or apathy levels are near maximum. The easiest way to raise her mania is to end the dialogue with a line like “It’s like talking to a wall,” or use medications.

Once the desired state is reached, Klara admits to stealing the morphine for her stepmother, Katerina Saburova. Head to the governor’s house in the Tanners’ District. In the large building behind the brick wall, examine the room on the left—the morphine is on the dressing table, which triggers a conversation with Katerina.

You’ll learn two important things from her: Griff supplied her with morphine, and there’s a certain Rat Prophet. Katerina will explain that he appears only at night in the Khrebtovka district, near the “stairway leading to heaven,” and will give you a bell to summon him.

Before nightfall, you can visit Griff in the Warehouses (if you rescued him on the fourth day). His lair is the building with the studded tires in the upper right corner of the area. Talking to Griff allows you to strike a deal and receive useful drugs, including morphine.

After 7:00 PM, head to Khrebtovka. The area is often contaminated, so act quickly. In the upper right corner, look for a staircase leading upward. At the top, you’ll find a bone altar—interact with it and ring the bell. The Rat Prophet will appear behind you.

During the conversation, he’ll offer a deal and reveal the idea for the “Rodent Extermination” decree, but ask you not to implement it. To preserve the possibility of further interaction with the prophet, agree or choose the “I’ll think about it” option.

Ending #4 – Academician

  • Achievement “Academician” : achieve an ending in which Bachelor Dankovsky doesn’t anger anyone.

This ending reflects the rational, scientific path of Bachelor Daniil Dankovsky, wholly rejecting the possibility of miracles and fables. To obtain it, one must not visit the cursed Yulia Lyuricheva Street in Stvorki, one must not enter the Polyhedron on the twelfth day, and one must not retain the knowledge of Simon Cain. If it was acquired earlier, it must also be erased.

Inquisitor Mark Karminsky must survive, and Aglaya Lilich’s actions must be stopped on the second day. However, the key decision in this case is the conscious choice to destroy the Polyhedron at the end of Pathologic 3.

Essentially, this path is identical to the ending of “The Price of a Miracle,” but with the opposite outcome: Daniil Dankovsky relies entirely on reason and science, recognizing the Polyhedron as a dangerous anomaly that must be eliminated.

This ending was bugged earlier in the release, but now it works correctly and can be achieved without any workarounds.

To clear the knowledge about the Cursed Street Walk, enter the command set_global_variable Know_BachelorWalkedDownTheStreet false in the console. To clear the knowledge about Simon Cain, enter the command set_global_variable Know_Main_YouAreSimon false in the console.

Ending #5 – Rot

  • Achievement “Rotten” : Achieve an ending where paralysis of will prevents clarity.

This ending is considered one of the darkest. The game literally punishes Dankovsky and the player, both morally and narratively. Formally, the central plot should be resolved, but it’s not the final choice that proves decisive, but rather the inaction leading up to the denouement.

The key condition is that the children remain trapped in the Polyhedron on Day 11. If Daniil Dankovsky doesn’t help them escape and then leaves the Pool on Day 12, the game records a tragic outcome for the children. In the finale, not only the children but also the Bachelor himself perishes.

 

It’s practically irrelevant whether Inquisitors Mark Karminsky and Aglaya Lilich are alive, or what path was taken previously. The important thing is that the Bachelor retains the ability to exit the Whirlpool on the twelfth day, and that the children remain inside the Polyhedron after day 11. This ending can be unlocked even after receiving the “True Immortality” ending, if you deliberately choose to ignore rescuing the children.

The game clearly interprets this outcome as a denial of responsibility—no attempt to intervene, no excuses, and no mitigating factors. If you didn’t do it, you get it!

Ending #6 – Fiasco

  • Fiasco Achievement : Never reach a single True Ending.

“Fiasco” is a complete failure of the main plotline. You’ll agree, that’s hardly a good word to describe anything. In this case, Bachelor Dankovsky not only makes the wrong choice, but also fails to complete the task for which he came to the City-on-Gorkhon in the first place.

To obtain it, you must fail the main storyline. In practice, this can be achieved by ignoring Aglaya Lilich and Artemy Burakh’s storylines, even partially, starting on the eleventh day. If the main storylines are not completed to their logical conclusion, the game automatically registers a failure.

In this ending, the Bachelor is unable to complete his research or make any meaningful decisions. After the mission’s failure, he loses his purpose and, essentially, the possibility of further existence or life.

If, for some reason, canceling the storylines doesn’t work correctly (which sometimes happens), the ending can be forced through the console. This resets key “flags” related to understanding the nature of the Polyhedron, the role of Simon Cain, and the origin of the Sandman. After this, simply speak with Inquisitor Mark before exiting the Whirlpool, and the “Fiasco” ending should activate correctly.

To use the console, enter the following commands:

  • set_global_variable Know_Main_RodOfPolyhedron false,
  • set_global_variable Know_Main_SimonInfectedTheTown false,
  • set_global_variable Know_Main_PolyhedronCausedThePlague false,
  • set_global_variable Know_Main_UnergroundBloodRivers false.

Ending #7 – Predestination

  • Achievement “Predestination” : See how Lyuricheva Street changed your destiny.

This ending is conceptually close to the ending of “The Academician,” but differs in its philosophical subtext. Here, Bachelor Dankovsky doesn’t make a conscious choice—the very structure of the world of the City-on-Gorkhon decides everything for him.

To achieve this ending, you must not visit the Polyhedron on the twelfth day and retain Simon Cain’s secret knowledge. Inquisitor Mark Karminsky must survive, and Aglaya Lilich’s deeds must be stopped on the second day.

The key and most difficult requirement is completing absolutely every character side quest. This is what makes the “Predestination” ending one of the most challenging in the entire game.

As a result, the Polyhedron will be destroyed, but not by the player’s will. Bachelor Dankovsky does not choose destruction—he simply follows the chain of events orchestrated by the “greater plan.” Rationality gives way to fatalism: the world comes to its inevitable outcome on its own, regardless of Daniil’s intentions.

Due to a bug at release, this ending was impossible to obtain correctly for a long time, but now it works reliably.

Ending #8 – Escape

  • Achievement “Escape” : Find the ending that Bachelor Dankovsky was originally looking for.

“Escape” is a completely alternative ending, not directly connected to the fate of the Polyhedron. It revolves around Dankovsky’s personal choice to abandon his role as savior and simply leave.

The first and most important condition is that Griff must be alive by the seventh day. He must be found in Isidor Burakh’s house. If Griff dies, the path to this ending is completely closed.

That same day, a chain of events involving the switchman and his wife is triggered. They need to find the carpenter’s grave, tell Laska about it, and then convince the parents that their son is alive. The phrase “he is in a superposition” is crucial in the dialogue.

 

Next, on the seventh and eighth days, you must negotiate with the Odong Gora in the steppe settlement of Shekhen about the upcoming escape—this also unlocks the “Master of Negotiations” achievement. On the ninth day, you must rescue Katerina Saburova, earning the “Seventh Seal” achievement, and then blackmail Alexander Saburov into assisting the escape and preventing the vigilantes from interfering.

On the tenth day, Dankovsky receives a letter from Griff. He must immediately go to Isidor’s house, discuss the escape, and agree. In his conversation with Maria Kaina, it’s important to absolve himself of responsibility for what’s happening in the city, denying personal guilt.

The final step is to cross the steppe to the locomotive and, in a conversation with the Mountain, confirm his readiness to leave. Thus, Dankovsky leaves the City-on-Gorkhon on the tenth day, neither saving nor destroying, but simply relinquishing all authority and declaring that this world cannot be healed.
